Reporting potential illegal acts
The company MEDIATE s.r.o., Company Registration Number: 64791866, with its registered office at Dolní Libchavy 325, 561 16 Libchavy, Commercial Register No.: C 9702, registered at the Regional Court in Hradec Králové (hereinafter referred to as the “Company”), hereby publishes the following information regarding the reporting of potential illegal acts in accordance with the provisions of Section 9, Paragraph 2, Letter b) of Act No. 171/2023 Coll., on the Protection of Whistleblowers.
Ms. Martina Kopecká (hereinafter referred to as the “Relevant Person”) was entrusted with handling the agenda related to the reporting of potential illegal acts. A notification of a possible illegal act can be submitted to the Relevant Person through one of the following internal reporting channels of the Company:
• electronically via the e-mail address whistleblowing@mediate.cz; this e-mail address was set up exclusively for the needs of the internal reporting system, however, it is still appropriate to include “WHISTLEBLOWING” in the subject line of the e-mail message,
by letter sent to the relevant person at the address of the Company’s registered office as registered in the Commercial Register; it is advisable to visibly state "WHISTLEBLOWING" on the envelope,
in person to the relevant person, who can be reached at the address Dolní Libchavy 325, 561 16 Libchavy on working days from 9:00 a.m. to 12:00 p.m. after prior arrangement
by phone to the relevant person at tel. number +420 725 172 430 on working days from 9:00 to 12:00
Potential illegal actions can also be reported to the Ministry of Justice via the reporting system at https://oznamovatel.justice.cz/.
Reporters can contact both internal and external reporting channels. However, they should use the internal reporting channel as a priority to submit a report. The report must include the name, surname, date of birth, and employment relationship with the obligated entity. Anonymous submissions will not be investigated. The employer has excluded from the circle of whistleblowers who are authorized to submit a report according to the internal directive through the internal channel persons who do not perform work or other similar activities for him.
